Patient profile
The patient profile is the record for one patient — their details, history, consultations, and related documents.
What’s in a profile
- Identity and demographics — name, date of birth, and identifiers.
- Clinical summary — history, medications, and allergies, as recorded.
- Consultations — the timeline of encounters. See Previous consults.
- Documents & history — related files and record activity. See Documents & history.
Creating a patient
Start a new patient
Create the profile before or during starting a consultation.
Enter core details
Add name, date of birth, and identifiers accurately — these are used to confirm identity and to attach clinical content.
Save and confirm
Double-check the details before you rely on the profile.
Confirming the right patient
⚕️
Always confirm identity with at least a second identifier (such as date of birth) before capturing or drafting. Selecting the wrong patient attaches clinical content to the wrong record.
Keeping records accurate
- Correct out-of-date details when you notice them.
- Don’t create a duplicate profile for an existing patient — search first.
- Keep identifiers accurate so records match across the clinic.
Privacy
🔒
Only open patient profiles you have a legitimate reason to access. Access is logged, and records are protected by clinic-level access controls. See Data security and Audit trail.
Common mistakes
- Creating a duplicate profile instead of finding the existing one.
- Entering an incorrect date of birth, which breaks identity confirmation.
Troubleshooting
- Can’t find a patient, or details look missing? See Missing patient context.
