Missing patient / context
Covers a patient you can’t find, context that looks missing or thin, and the important case of the wrong patient being selected.
Can’t find a patient
Symptom
A patient doesn’t appear in search.
Likely causes & fix
- Spelling or filters — check the spelling and clear any filters.
- Not created yet — the patient may not have a profile. Create one. See Patient profile.
- Access — you may not have access to that record. Check with your admin.
Context looks missing or thin
Symptom
The patient’s history, medications, or previous consults look incomplete.
Likely causes & fix
- It was never recorded — Miraa shows what’s in the record. Add and verify what you know.
- Imported context not added or not verified — see Clinical context.
- No integration — if you expected data from another system, that connector may not be live. See Integrations overview.
Thin context is a prompt to confirm with the patient, not to assume. Verify history and medications directly rather than trusting an incomplete record.
Wrong patient selected
Symptom
You realise a consultation, capture, or note is on the wrong patient.
This matters
Clinical content attached to the wrong record is a safety and privacy issue. Handle it deliberately.
Step-by-step
Stop
Don’t approve or send anything while the wrong patient is attached.
Correct the attachment
Move or re-associate the content with the correct patient using your clinic’s process. If you’re not sure how, ask your admin before proceeding.
Verify both records
Confirm the correct patient now has the content and the wrong record does not.
Note it
Follow your clinic’s process for recording the correction. Corrections are auditable. See Audit trail.
Confirm patient identity with a second identifier before capture to avoid this entirely. See Before a consultation.
When to contact support
Contact support if content is attached to the wrong patient and you can’t correct it safely with your clinic’s process.
What to include when reporting
- That it’s a wrong-patient or missing-record issue (describe without patient identifiers).
- What you were doing when it happened.
- Whether an integration was expected to supply the data.
